Homemaker Personal Care (HPC) supports individuals with everyday tasks so they can live safely at home and stay active in the community. Services are tailored to the person’s needs and goals and are delivered according to the ISP and waiver guidelines.

Home Care

  • Support with bathing, grooming, dressing, toileting, and other personal care needs

  • Assistance with meals, including preparation and eating support when needed

  • Help maintaining a safe, clean environment through light housekeeping related to the person’s care

  • Reminders, supervision, and help following daily routines

  • Support in the community for errands, appointments, and building independent living skills
